List of docs needed for Illinois IRP
1. ILLINOIS CDL or ILLINOIS DRIVER'S LICENSE
Registrant or the owner of the company must have Illinois CDL or Illinois driver's license. (If driver's license provided, CDL copy of the truck's driver will be needed.)
2. PHONE BILL
Phone bill is a MUST in the State of Illinois. Please provide copy of the phone bill (cell phone or landline) along with the page listing the actual phone number.
3. TWO ADDITIONAL PROOFS OF ADDRESS
Acceptable proofs of address: gas bill, electricity bill, water bill, waste bill, internet bill, cable TV bill, bank statement, apartment lease, mortgage statement, property tax bill, Articles of Incorporation, Illinois State Income tax copies
NOT ACCEPTED: credit card bills, auto finance bills, hospital bills, cancellation notices, social security statements, immigration papers
5. TITLE COPY FOR VEHICLE or ST-556 FORM
A copy of the front & back of the vehicle's title is needed or Sales Tax Form (ST-556) if the vehicle was purchased from IL dealer. Bills of Sale only accepted from out-of-state dealers (all signature areas must be signed & Dealer Number has to be provided)
6. Form 2290 (Heavy Highway Use Tax)
If vehicle was purchased more than 60 days ago Schedule 1 with IRS "Received" or "E-File" stamp would be needed. We can file it quickly if you don't have it.
7. Illinois IRP Application & Illinois Schedule G
Applications must be signed by the applicant.
8. Copy of previous registration
If applicable.
