Docs needed for Florida IRP

All titles must be in the name of the owner (either you or company you own). For vehicles titled in Florida, Florida title number may be used instead of the title copy. All out of state titles must have a copy of front & back and a completed VIN verification form. 

Certificate Holder: Florida DHSMV, 2900 Apalachee Pkwy, Tallahassee FL 32399 Must include PIP (Personal Injury Protection) $ 10,000 and at least $ 750,000 CSL (combined single limit coverage)

ACCEPTABLE PROOFS: Florida CDL or driver's license, bank statement (not older than 90 days), utility bills (gas, electric, water, not older than 90 days), property tax bill, FL vehicle title, corporate papers issued by Florida Division of Corporations.

PLEASE NOTE The following bills ARE NOT acceptable proofs of address: cell phone bill, cable TV bill, internet bill, insurance bill, credit card bills.

Lease must state who is responsible for insurance and who is responsible for safety. Must be signed and dated by the owner-operator and Motor Carrier's representative.

If vehicle was purchased more than 60 days ago Schedule 1 with IRS "Received" or "E-File" stamp would be needed. We can file it quickly if you don't have it. 

Form HSMV 85017

IMPORTANT: Signature on this form MUST BE NOTARIZED (you may notarize your signature at your local bank or Currency Exchange)

Required only if vehicle has an out of state title. 

Form HSMV 85900, Schedule A must be signed by the applicant.

Form HSMV 96440. Registrant or corporate officer must sign the POA in the presence of two witnesses AND a public notary (the notary may serve as one of the witnesses).
