Docs needed for Ohio IRP
1. COPY OF VEHICLE TITLE
Certificate of Title, Memorandum of Title, or Electronic Title Receipt. Photocopies are acceptable. The front and back copies of the title must be provided. If a truck is owned by someone other than registrant, OH Form BMV 4845 (Power of Attorney) will be required.
2. PROOF OF OHIO RESIDENCY
Three (3) proofs of residency are required. Acceptable proofs: Ohio driver's license or CDL, Federal Income Tax Return, Ohio State Tax Return, County Property or Real Estate Tax bill or statement, utility bill (gas, electric, water, sewer, phone-land line), Vehicle Registration and Title.
If incorporated, provide copy of Articles of Incorporation or Articles of Organization (for LLCs)
3. SAFETY CARRIER INFO
The USDOT # and Federal Tax Identification Number (FEIN) of the Motor Carrier Responsible for Safety (MCRS). If the registrant will be leasing on to a Motor Carrier's Operating Authority, they will also be required to show a copy of signed Lease Agreement or OH Form BMV-4885
4. OHIO IRP APPLICATIONS
Ohio Forms BMV 4890, BMV-4846, and PUC 3422 (Declaration of Knowledge for Commercial Vehicle Registration) must be signed and submitted.
5. HEAVY HWY VEHICLE USE TAX (FORM 2290)
Required for trucks purchased more than 60 days ago. This form is required for any truck with a registered weight of 55,000 lbs. or higher.
