Docs needed for New York IRP
1. COPY OF VEHICLE TITLE
A photocopy of a NY title in the registrant's name OR a photocopy of a Receipt for Title (FS-6T) in the registrant's name (form FS-6T can be obtained from any NYS Motor Vehicles office). A photocopy of an out-of-state title in the registrant's name and proof of sales tax clearance (sales tax clearance can be obtained from any NYS Motor Vehicles office). If the title is being held by a lienholder, a copy of the title in the owner’s name, certified by the lienholder or the out-of-state motor vehicle agency must be submitted along with the lienholder’s statement identifying the owner and the vehicle’s year, make and VIN. The statement must be an original, on the lienholder’s letterhead and must state that the lienholder holds the original title and is aware that the title copy will be used to register the vehicle in NY.
NOTE: IF THE REGISTRANT IS DIFFERENT THAN THE OWNER
If the registrant is different from the owner, you must provide authorization to register the vehicle by having the owner complete any one of the following for each vehicle: the “Registration Authorization” section on form IRP-6 or IRP-6A; Registration Authorization (form MV-95); or the “Different Owner” section on the Vehicle Registration/Title Application (form MV-82)
2. NEW YORK INSURANCE ID CARD
NYS Insurance Identification Card (form FS-20, FS-21, FH-1 or FS-77) in the registrant’s name (must be barcoded and scannable) OR a copy of a federal ICC/MC or NYS DOT permit in the registrant’s name.
3. PROOF OF NEW YORK ESTABLISHED PLACE OF BUSINESS
FOR INDIVIDUALS acceptable proofs: NYS Driver's License, utility bill (only one utility bill is accepted), current credit card or bank statement (not older than 90 days), mortgage document, homeowner's insurance document, real estate tax bill, NYS Certificate of Title.
FOR BUSINESSES acceptable proofs:
4. FORM 2290 HEAVY HWY VEHICLE USE TAX
Proof of payment of the Federal Heavy Vehicle Use Tax (IRS Form 2290) for vehicles registering at 55,000 lbs. or higher.
5. NEW YORK IRP APPLICATIONS
A Schedule A & C (form IRP-6) and Schedule B (form IRP-5)
6. PROOF OF IDENTITY
Proof of identity for the registrant (and owner, if different from the registrant), or for the person signing with a power of attorney.
